When you arrive, our receptionist will verify your address, telephone number, employment, and insurance information. This is necessary so that we may contact you regarding your care and also to facilitate billing your insurance carrier. Please be sure to bring your insurance card to each appointment and to present your insurance card, medicare, medical assistance, or any other health care payer information to the receptionist at the time of the appointment.

Your insurance plan may also require a co-pay with your office visit. This is quite common. Since your co-pay is due at the time of your appointment, the receptionist collects co-pays at the time of registration.
